Insurance and Billing
Patients need to bring their insurance cards at the time of their initial appointment and are responsible for at least a $50 deposit toward their office charge. Patients who have not met their yearly deductible are responsible for payment of the complete charge for service at the time of their first visit. Patients whose insurance requires a copay are required by their insurance company to pay that amount at the time of service. If the patient is being treated for an injury resulting from an accident in which a third party is responsible for the payment (such as an auto accident), we require payment in full at the time of the patient's visit. For a patient injured on the job and approved by their workers' compensation carrier, we will bill the carrier for the patient's visit. If for some reason the company later denies responsibility for the bill, the patient will be expected to pay for services rendered. If surgery is necessary, insurance benefits will be discussed with the patient prior to surgery. If deductibles and/or out of pocket expenses have not been met, a surgery deposit will be required. For your convenience we accept Visa, Master Card and American Express.
